SSA-1099 box 5 is $68,406 (of which is $3,876 for 2012; $15,812 for 2013; $16,056.00 for 2014; $16,332 for 2015) does it all count in 2016?

Yes, if you received it all in 2016 it's all taxable in 2016.  The fact that some of the benefits relate to prior years doesn't change that fact that you received the payments in 2016.
