Simple IRA Withdrawal - WHERE Do I Put It?

I have wasted so much time looking for what SHOULD be a simple thing. I understand that we do not file or need IRS Form 5498 for withdrawals made on a Simple IRA account. Taxes were taken out of that money that was withdrawn and the only thing I want to know is WHERE to put those numbers? Since I have the year end statement, I have the figures, just cannot find where to put them. Everything I found on here just says you don't need to file  Form 5498 but never bothers to add a simple WHERE to put the information??

For a SIMPLE IRA withdrawal, you should have a Form 1099-R from the company.  many companies allow you to get copies of the information online.  

This information is entered in the Income section for Retirement Plans and Social Security under IRA, 401(k), Pension Plan Withdrawals (1099-R).
