Simple IRA - I can't take out and I have not paid any funds too since 2015. Do I have to report it on my 2016 taxes and if so where do I report since I don't have a form?

Simple IRA I am not putting funds into that account, nor is my employer. The simple IRA has not had any contributions since 2015. Do I need to report it in my 2016 taxes?

You do not need to show this anywhere on your taxes as no contribution was made in 2016, nor was a distribution received in 2016.

If you received a Form 5498 that is an information document to be kept with your tax records.
