Shouldn't the pension amount received be listed on line 4c as well as line 4b (taxable amount)? It is not on there. also line 4a is erroneously blank!

When all of your retirement distributions are taxable, the total amounts goes on 4b for IRAs and 4d for pensions and annuities.

 

This is an excerpt from the line instructions for Form 1040:

 

"Except as provided next, leave line 4a blank and enter the total distribution (from Form 1099-R, box 1) on line 4b."

 

and later:

 

"If your pension or annuity is fully taxable, enter the total pension or annuity payments (from Form(s) 1099-R, box 1) on line 4d; don’t make an entry on line 4c."
