should I report 1099 R for a cost basis lost classified as a distribution when I closed an employer Roth 401k? No value in Box 1, Gross Distrib - only box 5 Roth Contrib
Have entered under wages/income - but final review throws error due to no value in Box 1 Gross Distribution. So do I even need to include since was just rolled into another Roth IRA?

You do not have to report the 1099-R with the Roth 401k cost basis information (Box 5) only. You can delete the entry from TurboTax so that you can e-file the return.
