Should I change downloaded name and address to agree with received 1099-R?

Downloaded form contains all correct information (including payer's federal ID#), except received 1099 has agent's name and P.O. box address.

You do not need to change your imported 1099-R information (unless the 1099-R you received in the mail is marked as a "corrected" copy).  One of the most important items that must match is the EIN (which you already said is correct).
