rolling over an annuity cash out i to an IRA

Hi,  I cashed out an annuity from my old investment firm having no taxes withheld and immediately rolled over into an IRA.  How do I report my Form 5498 so as not to incur taxes from my annuity?  I am reporting my 1099R from the annuity.  Thank you for your help.

After you enter the 1099-R information, you must continue to the follow-up questions.
