Public Service Officer benefit for retired law enforcement. Take up to $3,000 off taxable income if insurance benefits are deducted from your annuity.

Is there an IRS form for this credit?

It's not a credit, it's an exclusion from income available to retired Public Safety Officials.  Turbotax will ask if you are a retired PSO when you enter the Form 1099-R.  See the screenshot below.
