On the turbo tax form that deals with 1099R, Box 15 is State number. where do I find this number? Or, how do I get past this page

If you do not have an amount in Box 14 (State tax withheld), leave box 15 and 16 empty.

I left both box 14 and 16 empty, and the state number is still required to move past this page. 

You could enter the Payer's TIN in Box 15.

Are you saying that there is no Payor's TIN on the far left of the 1099-R, second row, just to the left of your Social Security number?
