On my wife's Statement of Annuity Paid, Box 2a Taxable Amount has unknown written in it. How do I record that on my tax form?

That is not uncommon for an annuity.  Just leave box 2a blank and enter the rest off the information on the Form 1099-R and the program will determine the taxable amount based on the 1099-R and your answers to the interview questions.
