On a 1099R form the Name of Locality (Box 16) has incorrect county. It says Daviess and should be Monroe. Do I match the 1099 R form, or put in our county Monroe?

It is recommended that you enter the 1099-R "as is".  Therefore, it is recommended that you enter Daviess.
