On a 1099-R Box 7, it asks for a distribution code but only allows one to select one. So I can't put "4D" in it. (I have to select either 4 or D.

You can enter the 4 and the D in separate boxes in TurboTax. On the 1099-R entry, there is a box you can check so that you can make two entries.
