My UC-1099G form for PA doesn't have these specified boxes (Box 1-11). I don't know what my state, federal, or local tax withheld is. Do I have the wrong form?

TurboTax is asking for information for Box 1, 4, 10a, 10b, 11, and Local income tax withheld. The form I was provided only lists Social Security #, Total Payment, Tax Withheld, Tax Year.  So I'm not sure where to enter my tax withheld or state identification number is.

Each state does 1099-G forms differently and they may not have the same corresponding box numbers as TurboTax.  You can enter your SSN and the amount paid and then check the box for withholding.  

 

Assuming the 1099-G was for unemployment just enter the information that does appear on your 1099-G:

  1. Enter the unemployment compensation amount in Box 1
  2. Enter the federal income tax withheld in Box 4.
