My income is from my 401k and annuities , do I use deluxe or premier version of TurboTax?

The Free edition supports entry of a Form 1099-R for the 401(k) withdrawal and the annuities also on a Form 1099-R.  However, if this was an early withdrawal from the 401(k) then you will need the Deluxe edition.
