My Cost Basis is missing on my 1099-B form for one of my items. Do I just park it down as $0.00?

It is just missing on my form outright and I can't seem to write down anything else other than $0.00

Your cost basis is what you paid for the stock (or other investment).

 

It is not unusual for your broker/financial institution to leave this field blank. 

 

You can enter that amount in the 1099-B interview, even if in the Review/Edit mode.
