They then need to issue a correct original code G Form 1099-R for this distribution to the obviously alive participant. If they refuse, you'll need to enter a substitute code G Form 1099-R and provide explanation.
Code 4 is for a death and code G is a direct rollover....was this due to a death?
Not a death I just rolled over my personal 401 from former employers to an ira
