My 1099 R form does not have information in boxes 12 to 14 and it does not say exempt . What do I do ?

The issuer should never put anything in box 12 other than a dollar amount.   If it says exempt they were probably trying to tell you that there is no state tax so leave all boxes 12-14 blank.   Nothing shroud be in any box if no tax withheld in box 12.
