If my gross distribution was not actually distributed, do I need to get a revised 1099-R, or complete any other forms?

No, don't necessarily need to get a corrected 1099-R form. After you enter you 1099-R, answer the subsequent questions as to what the 1099-R form was actually for.
For example: The money distributed was rolled over into another retirement fund.

The payer would generally not issue any Form 1099-R if there was no distribution. Why do you believe that there was no distribution?
