if my 1099-R Traditional IRA) shows: Box 14 (state W/h): 69.08 Box 15: OH but NO payer's state no. Box 16 (state dist): 0.00. I need payer's state # & Box 16 to file.

Box 14 (State Withholding) : $69.08 Box 15( State / State no): OH / (MISSING) Box 16 (State Distribution): 0.00 I cannot submit my return without the payer's state number and the state distribution amount. Should i use the EIN and just take the total distribution amount from 2A?

Use the federal EIN and 2a amount.
