If I take a loan from my 401k that is less or the amount I can borrow do I have to report it to the IRS?

A loan from a 401(k) is not reported on a tax return.  If the loan is not paid back or you leave the company without paying back the loan, then the loan becomes a distribution and is reportable as taxable income on a tax return
