If got both a 1099-K form and a 1099-NEC from my employer, do I need to include both those forms as part of my income?

I'm not sure.

I suggest you contact your "employer".  Neither of these are employee forms.  They are for a private contractor and a third-party payee.

If you don't get a W-2 you will be required to pay self-employment taxes.

The self-employment tax rate is 15.3%. That rate is the sum of 12.4% for Social Security and 2.9% for Medicare.
