I took a Covid-related distribution from my retirement account in December 2020 - do I need to wait for Form 8915?

If you selected to spread the distribution over three years, Yes you have to wait for the Form 8915-F.  If you did not spread the distribution over three years then you do not need to wait and you should have selected NO when asked about any 2017 thru 2020 Disaster Payments.
