If you receive a 1099-R from the plan, you must report it, although it should not be taxable if you did the rollover correctly.    For some types of rollovers, you won't get a 1099-R, and those types of rollovers do not need to be reported on your return.

Yes, a rollover of a 401(k) is always reportable by entering into TurboTax the Form 1099-R that you receive for this transaction.
