I rolled over 100% of my 401K to a IRA. I received a 1099-R for the distribution and 5498 IRA contribution form. TurboTax only asked about the 401K distribution.

How do I indicate that this was a 100% rollover and should not be taxed?

If there is a code G in box 7, it is already coded as a rollover and you don't need to do anything else. If not, on one of the next few screens you will be asked what you did with the money. Check that you rolled it over into another retirement account. See the screen shot below.
