I repaid SSA benefits in 2018, but SSA-1099 shows box 5 as receiving what was paid back, box 3 is twice what was received, why?

Box 3 is twice the amount actually received.  Box 4 (repaid) is what was received and returned.

But box 5 shows a value equal to what was received, I would expect this to be 0 (as all was refunded).   Description shows amount paid, and an item for Deductions for work or other adjustments of the same value.  which totaled doubles the amount

Actually, after finishing entering all my tax info, the extra tax generated by this went away.  So now mostly curious as to why they report twice the value received.

You have to ask the Social Security Administration to explain it. (It might be a mistake.) Call them at 800-772-1213, Mon - Fri, 7 a.m. - 7 p.m.
