I RECEIVED TWO 1099r FROM ONE ANNUITY, EACH WITH A DIFFERENT ACCOUNT NUMBER. DO I COMBINE THE AMOUNTS OR ENTER EACH SEPARATELY?

You should enter each 1099-R separately. You can add that additional 1099-R by following the instructions below:
