I received a "wellness" refund from my emp that reflects a portion of my insurance premiums being refunded. Where would I report that? They are not providing a tax form.

It's reported as Miscellaneous Income.

To get to that section in Desktop:

1.  Click on Federal Taxes across the top.

2.  Click Wages and Income.

3.  Click I'll choose what I work on.

4.  Scroll down to Less Common Income.

5.  Click Start or Update by Miscellaneous Income, 1099-A, 1099-C.

6.  Click Start or Update by Other Income not already reported on Form W-2 or Form 1099.

7.  Click on Other Earned Income.

Before you do this, though, ensure that the income isn't already included in your wages.  If it is, you've already reported it in the W2 section.
