I received a distribution from my 401h account held by my former employer. Will i receive a 1099 from my former employer

My former employer contributed monies to a 401h account years ago. I withdrew money from that account to pay for COBRA premiums.

Yes, you are due a 1099-R for the distribution.

Per the IRS and Empower Retirement Company (www.empower-Retirement.com)  They do not provide 1099-R because this is an after tax paid benefit.  https://www.irs.gov/pub/irs-pdf/p11433.pdf
