I received a 1099R from John Hancock - I rolled over all money to Vanguard. No money was taken out. Do I need to disclose this? If so where on tax form?

Yes. There are two lines on your tax return - the gross and taxable amounts, so report your rollover so it matches with IRS records. Otherwise, they may question whether you actually rolled over the money.
