I received $2059.02 from Allianz Life Insurance this year, probably the same each year of my retirement. Is this a 403b or 401k or what?

You would need to contact the company as we do not know why you are receiving it or the details of the plan or what it is for. 

@rothll curious, why does it matter? the tax treatment is the same. When you receive the 1099-R form, look at the checkbox next to Box 7.  That will indicate whether it is or is not an IRA.
