I pulled 3000.00 from my 401k as a loan how do y’all add to my taxes and do I need a paper from my employer

A loan from a 401(k) account is not reported on a tax return.  Only if you default on the loan, then the distribution would be on a Form 1099-R and entered on a tax return.
