I paid states taxes on my Ohio Public Employees Retirement but can't find where to place it on this form

It depends on the tax form that this was reported on and if you received a distribution from your retirement account. This is usually reported on a 1099-R in box 14.

We've had another OPERS reports that they issued 1099-R forms with boxes numbered using the old #system,...and they haven't updated their 1099-R forms to the proper 2020 format.

So OPERS 1099-R

Their box 12 goes,  into TTX box 14 

Their box 13 goes,  into TTX box 15 

Their box 114 goes,  into TTX box 16   (if their box 14 has a value...many times it's left empty and either box 1 or box 2a are used automatically instead)
