Enter your ssa-1099R, but you don't enter the Medicare payments there.

 

You enter the Medicare amount you paid in the Deductions & Credits section.

You don't enter it on a form even if it listed on your ssa-1099-R

You need to go to the screen that lets you enter the amount 

 

Click Federal

Deductions & Credit 

Scroll down to Medical Expenses

Click Edit or Start

 

Go through the screens until you get to "How much did you spend on insurance premiums?'

and enter there
