I opened a Roth IRA in 2017 and made contributions. Do I fill out both the "Traditional and Roth IRA Contributions" and "Retirement Savings Contribution Credit" sections?

Yes, although you will not get a deduction for your Roth contributions, entering the information will track basis information.

In addition, your Roth contribution may qualify you for the Retirement Savings Credit.
