I moved traditional IRA's to a different custodian. 2 of the 4 investments are on 5498's from both custodians and the one I moved from insists they are correct. Help!

Is there a way to indicate the correct amount on my return?

You don't need to enter Form 5498 on your tax return. Rollovers are reported on Form 1099-R but if it was a direct rollover then sometimes you don't get Form 1099-R since it isn't a taxable event. You might want to check with your custodian.
