I have received form 5498 regarding IRA information. Is there somewhere on my tax return that I need to input this information - box 5 fair mkt value of acct?

No, you don't need to enter Fair Market Value of your IRAs on your tax return. Form 5498 is for informational purposes only and is not reported on your income tax return.  The form reports your annual IRA contributions, defines the type of retirement account you have, it also reports any rolled over or transferred amounts from different types of retirement accounts. Keep if for your records, please.
