I have my Fidelity Retirement Plan Statement but not tax forms. I took out 8K this past year and I am paying it back monthly with interest. Where do I list this on my tax

I don't know if this is a 401K or IRA

IF you took out money structured as a loan....and are paying it back according to the schedule you set up with Fidelity...then there is no tax form entry to make.  There will only be something to report if you default on your payments.
