I have an employer sponsored 401K. Should I have received a 1099 for contributions and should I put that on my tax forms somehow?

You will not receive a form 1099-R for your 401(k) contribution.

The amount you contributed is reported in Box 12 code D of your form W-2. This amount has already been excluded from your taxable wages reported in Box 1 of your W-2.

You do not need to report anything for this contribution on your tax return.
