I have a 5498 2016 Form 5498 IRA Contribution Information and do I need to put this into my tax information?

No.  You do not need to enter information from this form.  It is for your information.

Keep Form 5498 with your records for future reference when you need to determine the end of year value or your investment basis in non-deductible or Roth IRAs.
