I have a 1099R, box 7 is code 7. The distribution is 4001..25 which was paid as both federal and state tax. This shows as income not tax payments.

The distribution is indeed income.  If you had federal or state tax withheld on the amount, those withholdings are entered in different boxes on the form.  The amounts will show as both income and tax withholdings.
