I have a 1099-R from Raymond James. Our advisor had us convert some money from a traditional IRA to a Roth IRA. How do I report this and the taxes withheld?

You can enter the 1099-R under Retirement Income.  Because it was a conversion to a Roth IRA, you will have to pay taxes on all of it.
