I had a distribution from a deferred pension plan. They deducted Federal Tax, Medicare and SS. How do I report this? 1099 or W2? Many Thanks.

You should be receiving a W-2 for this showing amounts in boxes 1, 2, 3, 4, 5, 6 and 11.
