I do not have a 1099-G form, but I need information from it. How can I get this information?

The form must've gotten lost in the mail while I was moving. The mail has been extremely unpredictable for me this past year.

Google the state unemployment office where you filed for benefits.  Most have them available online, using the same system you used to apply for the benefits.  See Unemployment.
