I did a 401k to IRA Direct Rollover. What do I check for the Michigan verification section for "Where is this distribution from?"

I switched jobs and did a direct Rollover from my 401k to an IRA.  I got a 1099-R with the code G and entered all the information in TurboTax.  It is now asking me to verify a bunch of stuff after entering the info from the form. My state is Michigan and I think this pertains to that.  It is titled "Where is this distribution from?"  It asks for the Michigan distribution amount which in box 16 is zero on my form and I also have nothing in box 14.  It gives me a bunch of options to check.  I don't know if I should check "pension/ira distributions received before retirement age" or "none of the above".  I can rule out the other options.

 

Earlier in the double check it also said I left box 2a and 2b blank which I did not.  I put 0.00 in 2a which is what my form said and checked Total Distribution in spot 2b.
