I contributed to IRA less than I reported on last year tax. How I make a correction on this year tax?

On the 1040 I reported contributed $3,000to IRA, then later in June I withdrew from IRA $1,500. How do I make correction on this year tax?

You should have been sent a 1099-R for the $1,500 your withdrew in 2017. That information is entered under the Retirement Plans and Social Security section. It sounds like this would be considered an early withdrawal.
