I contributed for Traditional IRA in Jan 2018 for 2017 tax year & waiting for Tax Form from Fidelity. Can I do Tax return before Apr18 or need tax extension from IRS?

Enter the traditional IRA contribution into TurboTax based on your own records.  No need to wait for the Form 5498.
