I closed out my 401k in Florida last year. Do I need a 1099 or some other document to file my taxes?

Yes---you need a 1099R from the plan----they might mail it or you might need to go to their website----but the 1099R for the money you took out of a 401k must be entered on your tax return.

 

To enter your retirement income, Go to  Federal> Wages and Income>Retirement Plans and Social Security>IRA  401 k) Pension Plan Withdrawals to enter your 1099R.
