I changed jobs and transferred my old 401k to the new 401k both held at Fidelity. Do I receive a 1099-R so I can put this on my tax return?

Check with Fidelity, but I believe that, as long as this was a direct rollover of a 401k into a new 401k, it does not get a 1099-R.

If these were independent employers, a Form 1099-R is probably required and should have code G in box 7. Your Forms 1099-R are available for download from the Fidelity website.
