I am filing a paper copy of my state return. It is asking for a 1099r. I am unsure how to fill this out or where to obtain it.

Did you have retirement income?  If so, then you should have received a 1099-R from retirement fund administrator.  You would have had to include this income on your Federal return.

If you did not have retirement income, you would not have a 1099-R.
