The distribution is from the bank. Enter the name of the bank for Who paid you?.

THANKS, PATTI, BUT I'M REFERRING TO THE PART WHERE THEY WRITE THE FOLLOWING AND I HAVE TO CHOOSE:

Where is your distribution from?
Select the source of this distribution and enter any other New York information.
Retirement distributions from:
  • US Government (including military)
  • New York State and City Education
  • Certain public authorities
  • NYC Variable Supplemental Funds (VSF)
  • Certain annuity funds
  • Other Pensions of New York State and Local Governments
Retirement distributions from:
  • New York State and City Education (optional retirement program)
  • IRA rolled over from a former government pension
Not eligible for exclusion for those over age 59 1/2
None of the above (select for deferred compensation 457 plans)

If it is not a pension, select the last one (None of the above). Check to see if the box next to IRA/SIMPLE/SEP is checked. Ask your parents what distribution it is. Did either/both of them retire from NYS/NYC government? It all depends on what kind of retirement account the money is from.

Thank you. They did not work for government. I checked that box and moved on. Not sure where it is from because my father always did his taxes by himself but now has memory loss and I have to help him. He can't recall that information. Thank you so much for helping!
